I get asked constantly why our BMI tool shows different "normal" ranges for different regions โ this article is the explanation I wish existed before I had to research it myself while building the calculator.
Every year, millions of people calculate their BMI and feel confused, worried, or dismissive of the number. A professional athlete is told they are "overweight." A thin person with dangerous visceral fat is told they are "normal." A doctor uses it as the primary screening tool. BMI is everywhere yet very few people understand what it actually measures โ or what it doesn't.
Body Mass Index was developed by Belgian mathematician Adolphe Quetelet in the 1830s โ not as a medical tool, but as a way to describe the statistical distribution of body sizes in populations. The formula is simple: BMI = weight (kg) รท height (m)ยฒ. It lay dormant until 1972, when physiologist Ancel Keys published a landmark study of over 7,400 men across five countries, concluding BMI was a better measure of body fatness than other available methods. The WHO adopted it globally in 1995.

Muscle is denser than fat. A highly muscular person can have a BMI in the "overweight" range while having very low body fat. Cristiano Ronaldo has reportedly had a BMI above 25 despite exceptional fitness.
It ignores fat distribution โ the most important factor. Visceral fat (around internal organs) is strongly associated with metabolic syndrome, type 2 diabetes, and cardiovascular disease. Subcutaneous fat carries far lower risk. A "normal" BMI can hide dangerous visceral fat โ a condition called "metabolically obese normal weight."
It doesn't account for age, sex, or ethnicity. Body fat naturally increases with age. Women carry more fat than men at the same BMI due to reproductive physiology. The original thresholds were derived from European populations.
Waist circumference is one of the strongest predictors of cardiometabolic risk. WHO action thresholds: above 94 cm for men, above 80 cm for women.
Waist-to-Height Ratio (WHtR) โ divide waist circumference by height. A ratio below 0.5 is generally healthy. This accounts for body size and strongly predicts cardiovascular disease.
Blood markers โ fasting blood glucose, HbA1c, lipid panel, and blood pressure give a comprehensive picture no weight-based metric can provide.
Use BMI as one data point among several, not as the final word. If your BMI is in the normal range but you are sedentary with a large waist, you are at real risk. If your BMI is "overweight" but you exercise regularly, have good blood markers, and a healthy waist ratio, you may be in excellent health.
The most effective approach is to use BMI alongside complementary measurements. Waist circumference is one of the strongest single predictors of cardiometabolic risk โ the WHO recommends action thresholds of 94 cm for men and 80 cm for women. Waist-to-height ratio (divide waist by height; aim below 0.5) is increasingly preferred by researchers because it adjusts for body size. Blood markers โ fasting glucose, HbA1c, triglycerides, HDL cholesterol, and blood pressure โ complete the picture that BMI alone cannot provide.
If your BMI sits in the overweight range but you exercise regularly, eat a balanced diet, and have healthy blood markers, you may be at lower risk than your BMI implies. Conversely, a normal BMI with a large waist, elevated fasting glucose, and sedentary lifestyle represents genuine metabolic risk. BMI is a starting point for a conversation with your doctor โ not a verdict.

Critics note that Keys' original dataset was all male, predominantly White and European. Despite these limitations, BMI was adopted globally because it requires only two measurements โ weight and height โ making it enormously practical for large-scale population screening.
Muscle tissue is approximately 18% denser than fat tissue. A person with high muscle mass and very low body fat will have a higher BMI than a sedentary person of the same height. Professional athletes, bodybuilders, and regular weightlifters routinely fall into the "overweight" BMI category despite having excellent cardiometabolic health. BMI provides zero information about body composition โ the ratio of fat to muscle that actually determines health risk.
Multiple large studies have consistently found that South Asians and East Asians have higher body fat percentages at the same BMI as European populations, and develop type 2 diabetes and cardiovascular disease at lower BMI values. India's National Diabetes, Obesity and Cholesterol Foundation recommends 23 kg/mยฒ as the overweight threshold and 27.5 kg/mยฒ as obese for Indian adults โ significantly lower than the global WHO cutoffs of 25 and 30.
Waist-to-Height Ratio (WHtR) is gaining researcher support as a more reliable cardiometabolic risk predictor. Divide your waist circumference by your height (both in the same unit). A ratio below 0.5 indicates low risk regardless of BMI. WHtR directly measures abdominal adiposity โ the visceral fat around organs that drives metabolic disease.
Waist circumference alone is also highly predictive. WHO action thresholds: above 94 cm for men, above 80 cm for women. These are the thresholds above which metabolic risk begins rising significantly, regardless of BMI category.
Blood markers complete the picture: fasting blood glucose, HbA1c, lipid panel (LDL, HDL, triglycerides), and blood pressure. A normal BMI with elevated fasting glucose and a large waist represents genuine metabolic risk. Conversely, a "overweight" BMI with normal blood markers and regular exercise may represent low risk.
BMI Prime is the ratio of your BMI to 25 (the upper boundary of the normal range). BMI Prime = BMI รท 25. A BMI Prime below 1.0 means you are in or below the normal range. Above 1.0 means overweight or obese. This allows direct comparison across different BMI values without remembering category thresholds โ a BMI Prime of 1.2 means your BMI is 20% above the normal upper boundary, regardless of your height.
